IN CONSIDERATION, of being permitted to attend any one of the facilities or programs owned and operated by Cambridge Group Inc. o/a Cambridge Group of Clubs (herein "Cambridge"), I hereby release and forever discharge each of Cambridge and its employees, officers, directors, shareholders, agents, and other representatives (the "Releasees"), of and from all claims, demands, damages, costs, expenses, actions, and causes of action with respect to death, injury, loss, or damage to my person or property howsoever caused, arising directly or indirectly from my attendance at such facility or program, wherever situated, at any time or times after the date hereof, whether as spectator, user, or otherwise, notwithstanding that any such claim may have been contributed to or occasioned by the negligence of any of the aforesaid Releasees. This guest release forms the entire agreement between the parties. Any verbal discussions had with the participant or the family of the participant are not binding.

I FURTHER UNDERTAKE to hold harmless and indemnify the Releasees from and against any and all claims, demands, damages, costs, expenses, actions, and causes of action and liabilities arising as a result of or in any way connected to my attendance at such facility operated by Cambridge at any time or times after the date hereof.

I ACKNOWLEDGE the inherent risks related to my involvement in the practice of sports and/or physical training and fully accept any and all consequences related to these risks.

I ACKNOWLEDGE having read and understood the terms and conditions hereof, which shall be binding on me, my legal personal representatives, heirs, and estate trustees, and ensure to the benefit of each of the Releasees and their respective successors and assigns.
